Abstract

NB: this is a voluntary notice in relation to a contract for Part B services This is a commissioned service for adults and children, and their carers, who are vision impaired and are eligible for social care support and are resident in Denbighshire. The service will provide assessment, information, advice and short term support enabling eligible individuals to maintain their independence in the community. The service will be available 5 days a week, Monday to Friday during standard office hours 9am to 5pm In delivering the service the Provider will: - Carry out assessment of user needs through discussion and observation with the services user and appropriate others and within agreed timescales - Provide a rehabilitation programme negotiated with the individual funding body and appropriate others through teaching, communication, mobility and daily living skills to enable the individual to achieve and maintain maximum independence The provider will deliver a service which comprises the following elements as a minimum: - Rehabilitation Officer The Rehabilitation Officer’s role is to: - Provide a comprehensive assessment, rehabilitation including equipment, counselling and advice service to the visually impaired, and their carers. - Liaise with statutory and Voluntary Organisations, including Health, and advise on the provision of available services. - Rehabilitation Assistant The Rehabilitation Assistant’s role is to: - Promote independence - Enhance Service User’s quality of life by providing practical help, e.g., equipment demonstrations - Provide support to Service Users CPV: 85300000.
