Short description of the contract or purchase(s)
Conwy County Borough Council is inviting expressions of interests from suitably qualified and experienced Organisations to implement an electronic care monitoring system within social care in-house services in Conwy. The electronic care monitoring system is expected to deliver significant benefits across Conwy Adults Social Services including achieving efficiencies and improvements in all aspects of service planning, delivery, invoicing, reduction in back office functions and associated green benefits through the change from paper based system.
The new system will be an essential part of the outcome focussed care planning process, improving the Service User's involvement and experience in care planning and monitoring.
