NHS Wakefield Clinical Commissioning Group (WCCG) is seeking to establish a contract with one experienced and established consultant for its Care Closer to Home requirement.

Wakefield CCG wants to look at the difference in outcomes experienced by Wakefield residents who experience care through the integrated care teams and compare this cohort with those who haven't experienced integrated care.

The health and social care community in Wakefield wish to put people at the heart of their services and therefore want to carry out meaningful and ongoing patient and public engagement.

Therefore, Wakefield CCG wishes to commission an independent, high-quality, evaluation.
