Abstract

NHS Kernow Clinical Commissioning Group is seeking responses from organisations that can provide:

a) a software package that delivers a Digital Shared Care Plan containing essential patient information, including preferences for their palliative care and be easily accessible and updatable by the patient and the professionals involved in their care; and

b) a platform for an Electronic Palliative Care Co-ordination System that delivers an end of life (EOL) register and that enables the sharing of the register by care professionals. It must be able to show by individual, a single shared care plan, providing a consolidated view of care packages from disparate care settings and source systems; pulling information from core clinical and care IT systems and/or databases across primary care, secondary care, urgent care, mental health, community services, care homes and hospices. The system must be able to join, extract data and provide read only access.

Potential providers will need to demonstrate they are able to build the above mentioned, to implement the system, to mobilise on a phased approach and to offer and a comprehensive marketing/education programme to those using the system.

NB: This Contract Notice details a project funded through ETTF and will depend on successfully securing ETTF funding; the funding for this is not guaranteed.
